What is Support
Coordination?
Support Coordination is funded by the NDIS and is designed to assist participants to understand and use their NDIS plan.
Support Coordination is a capacity building support which helps you to:

Understand and use your NDIS plan

Pursue, implement and achieve your goals

Connect you with NDIS providers, community, mainstream and other government services

Build your confidence and skills to use and coordinate your supports.
A Support Coordinator delivers support coordination services and they will assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
If you have been funded Support Coordination in your NDIS plan we can assist you. You will be appointed a Support Coordinator who is an expert in the delivery of Support Coordination and it is their role to help you navigate the NDIS and assist you to achieve your goals that you have developed with the NDIS.
Advance Support Coordinators will help you make the best use of your NDIS plan and connect you with other providers and services of your choice. We will support you to increase your independence and capacity, and ensure that you are connected to your community by developing relationships with other support networks.
Our team are all experts in this field and have a minimum of 3 years experience delivering Support Coordination to participants. Advance wants your experience with both the organisation and NDIS to be positive. They want to see you have choice and control but most of all they want to see you achieve your goals and deliver outcomes.
